Is it time for home care?

A quick checklist for families who are starting to worry about a parent or loved one.

Take 2 minutes

Check anything you have noticed

There is no score that decides for you. This list helps you put into words what you are seeing, so the next conversation is easier.

In an emergency, call 911. This checklist is general information, not medical advice.

A little help could go a long way

One or two of these signs often means a few hours of help a week would make daily life safer and easier. A free in-home visit is a good way to talk it through.

It's a good time to talk with us

Several of these signs together usually mean your loved one would benefit from regular support at home. Let's talk about what would help most. Your free visit and Plan of Care come with no obligation.

Talking with your loved one

Tips for the conversation

  • Pick a calm, unhurried moment, not right after a fall or a hard day.
  • Start with what they want, like staying in their own home, and how help supports that.
  • Talk about specific things you have noticed, not labels.
  • Offer a trial: a few hours a week to start, with a caregiver they meet first.
  • Invite them to the free in-home visit so they have a say in their Plan of Care.
